Disease Management Software

Primary PhysicianCare has an in-house software development team that customizes web-based applications to fit our onsite medical personnel's needs on an ongoing basis.  Thus, we are able to keep up with the latest trends in healthcare technology.

With this form, nurses are able to manage our members who have chronic conditions such as diabetes, asthma, and chronic hypertension.  Usually, this is done telephonically, but it also can be done at our onsite clinics.
